Pennsylvania Reports Fifth Measles Death as U.S. Case Count Hits 35-Year High
An unvaccinated Lancaster County resident died from measles as Pennsylvania's outbreak approaches 1,000 cases and the national total surpasses 3,600, the highest in three decades.
Pennsylvania tallied its fifth measles-associated death last week, deepening an outbreak that public health officials say is the worst the state has seen in more than a generation and is now driving the highest national case count in 35 years.
According to a statement reviewed by CNN, the Pennsylvania Department of Health confirmed that the person who died was an unvaccinated resident of Lancaster County. The department said it would not release additional identifying details but confirmed the death "meets the measles-associated death definition" and has been reported to the U.S. Centers for Disease Control and Prevention.
The four other deaths reported by Pennsylvania include two infants in Lancaster County, a 40-year-old woman in Jefferson County, and an unvaccinated 18-year-old in Mifflin County, according to CNN's reporting on the outbreak. All five deaths occurred in people who were not vaccinated.
As of September 30, Pennsylvania had confirmed 943 cases across 39 of its 67 counties, with roughly 40 percent of those concentrated in Lancaster County. The Hill reported at least 176 hospitalizations statewide. Cases in the state have more than doubled since August, according to Al Jazeera's coverage of the outbreak.
Lancaster County sits in the heart of Pennsylvania Dutch Country, home to large Amish and Mennonite communities with lower vaccination rates than the general population. The county's demographics matter clinically: measles moves fast in any population with vaccination gaps, and rural geography can delay the kind of early clinical contact that catches complications before they turn fatal.
The Pennsylvania outbreak is part of a broader national picture that was already alarming before this latest death. As of September 24, the CDC had confirmed 3,659 measles cases nationwide in 45 states and the District of Columbia, according to HealthDay's reporting cited by U.S. News. That figure exceeds the 2,289 cases recorded in all of 2025 and represents the highest yearly total in three and a half decades.
Public health experts quoted in reporting by The Washington Post noted that the real number is likely several times higher, because many people with measles never seek medical care and the CDC's confirmed-case count requires laboratory verification.
The federal count has become a source of friction between state and federal officials. The CDC's dashboard lists only two measles deaths for 2026. As The Hill reported, the agency changed its methodology to count only deaths where a death certificate names measles as the primary cause, rather than tallying deaths reported by states. Pennsylvania Gov. Josh Shapiro has clashed publicly with Health Secretary Robert F. Kennedy Jr. over that distinction. Kennedy has also publicly questioned whether some of the deaths were caused by measles, without providing evidence, according to AFP reporting carried by the Jamaica Observer.
For clinicians in the field, the methodology dispute is secondary to what the case counts already show. Measles is vaccine-preventable. The MMR vaccine is highly effective, and measles was declared eliminated in the U.S. in 2000. The CDC puts the child fatality rate from measles at roughly 1 to 3 per 1,000 infected, a figure that does not account for severe non-fatal complications including pneumonia and encephalitis.
The CDC has said it's ready to send disease investigators to Pennsylvania, according to an HHS spokesperson statement reported by HealthDay. Pennsylvania officials have also requested a formal Epi-Aid investigation, a tool used for urgent disease events, Al Jazeera reported.
Until vaccination rates recover in the communities driving this outbreak, the case curve is unlikely to change direction on its own.
