Pennsylvania Measles Outbreak Tops 1,000 Cases, the State's Worst in 35 Years

Five people are dead and 198 have been hospitalized as the outbreak, centered in Lancaster County, spreads across 39 counties, with more than 99% of cases in unvaccinated people.

Pennsylvania's measles outbreak has crossed 1,000 confirmed cases, the state's highest case count since 1991, and public health officials are still counting.

According to data released by the Pennsylvania Department of Health and reported by Outbreak News Today, the state confirmed 1,004 cases across 39 counties as of October 5, 2026. Five people have died. At least 198 have been hospitalized. The outbreak, which the Pennsylvania DOH says began in Lebanon County in late April, is now the largest measles outbreak in the United States.

The population hit hardest has been the unvaccinated. According to the DOH figures, fewer than 1% of cases involved people who had received the MMR vaccine. All five deaths and every hospitalization on record occurred in people who had not been vaccinated. That's a clinical fact worth sitting with: this virus isn't slipping past the vaccine. It's finding people the vaccine never reached.

Lancaster County, home to the largest Amish community in the country, is carrying the heaviest burden. NBC Philadelphia reported 391 confirmed cases there, nearly 40% of the state's total. Mifflin County is second with 118. The Observer-Reporter noted that Allegheny County, which includes Pittsburgh, logged its first case on Monday, an unvaccinated 18-year-old who is recovering at home.

Nationally, the CDC has recorded close to 4,000 measles cases this year, according to the Observer-Reporter. Measles was declared eliminated from the United States in 2000. What's changed is coverage. A Reuters/Ipsos poll conducted in September 2026 and cited by Wikipedia's outbreak entry found that 76% of Americans considered the MMR vaccine safe for children, down from 84% in a comparable poll in May 2020. That eight-point drop is not abstract. In a population-level disease with an R0 that can run as high as 18, small dips in community immunity create gaps that an airborne virus fills fast.

The state has been running pop-up vaccination clinics since the outbreak began. Outbreak News Today reported that DOH staff have administered more than 8,800 MMR doses through health centers and pop-up sites statewide in 2026. Demand is rising outside those clinics too: the same report noted Pennsylvania providers administered more than 53,000 MMR doses in September alone, compared with a typical monthly volume of roughly 25,000.

Health workers, the DOH noted, have been working specifically to reach communities that live more isolated lives and have historically had limited contact with routine vaccine infrastructure. What that means in practice, in Lancaster County, is going door to door and meeting families where they are. That's labor-intensive, unglamorous public health work. It's also the only kind that closes gaps the system missed the first time.

New York's rural counties declared a separate measles health emergency last week, a sign that this is no longer a Pennsylvania story. The question for state and local health officials across the country is whether the outbreak's momentum can be slowed before school-age vaccination windows close for another year.
