Cancer Therapy Puts Treatment-Resistant Rheumatoid Arthritis Into Remission in First Clinical Trial
A phase 1 trial at a Berlin hospital found that three of six patients with severe, drug-resistant rheumatoid arthritis reached sustained medication-free remission after a single CAR T-cell infusion.
A small but closely watched clinical trial has produced the first evidence that a therapy borrowed from oncology can push severe rheumatoid arthritis into medication-free remission -- at least in some patients.
Researchers at Charité -- Universitätsmedizin Berlin treated six adults whose rheumatoid arthritis had not responded adequately to any established treatment. According to results published in Nature Medicine on August 27, disease activity decreased substantially in all six participants. Three of those patients reached sustained remission and were off all rheumatoid arthritis medication by the end of the observation period, which ran up to one year.
The treatment is called CD19 CAR T-cell therapy. It works by taking a patient's own immune cells, genetically modifying them in a laboratory to seek out disease-driving B cells, and then reinfusing them. In rheumatoid arthritis, those B cells are central to the immune system's misdirected attack on the body's joints. According to reporting reviewed by ScienceDaily and MedicalXpress, PET-MRI imaging showed that inflammatory foci around participants' knee joints were no longer detectable several months after treatment, with reduced swelling and improved mobility.
The therapy isn't resetting everything blindly. According to a trial summary published on EurekAlert, the self-directed B cells that had been present before treatment were no longer detectable in nearly all patients after infusion, but protective antibodies from prior vaccinations -- including those for chickenpox and tetanus -- remained intact. That's a meaningful distinction for patients and for the rheumatologists who would eventually have to counsel them.
Still, the investigators are being careful about what these numbers mean. The trial's primary focus was safety -- specifically the incidence and severity of cytokine release syndrome and immune effector cell-associated neurotoxicity syndrome in the first four weeks. As reported in Nature Medicine, responses also varied: some patients didn't achieve a complete response, and in one case the disease returned after an initial medication-free period.
Gerhard Krönke, MD, who leads the joint Clinical Rheumatology research group at Charité and the German Rheumatology Research Center, called the results "particularly remarkable" given that none of the established treatments had previously relieved these patients' symptoms adequately, according to reporting reviewed by Genetic Engineering and Biotechnology News.
Rheumatoid arthritis is a chronic autoimmune condition in which the immune system attacks the body's own joints, causing recurring inflammation, swelling, and eventual joint damage. Traditional disease-modifying drugs and biologic therapies attempt to suppress that inflammation systemically, but for a subset of patients they don't work well enough. Those are the patients this trial was built for.
CAR T-cell therapy for autoimmune disease remains firmly experimental. There's no long-term safety data, no regulatory approval outside of oncology indications, and no roadmap yet for what the manufacturing cost structure looks like at scale. Six patients is a proof-of-concept, not a practice-changer.
But for a disease where the current ceiling for treatment-refractory patients is lifelong immunosuppression with diminishing returns, a phase 1 result showing that the immune system can be retargeted and -- in some cases -- apparently rebooted is worth taking seriously. Larger trials are the next step.
